Ongoing Support

Keep using the app to message providers, track goals, and access on-demand tools. Many patients continue with booster visits or coaching even after graduation.

Week 1

Meet with a patient engagement specialist, share your story, and complete your intake with a pain medicine provider. Together, we’ll begin building your personalized care plan.

Month 3

Your team checks in on progress, refines your care plan, and helps you stay on track. Whether you’re building new habits or navigating setbacks, we move forward together.

Graduation

Once you’ve built the skills and support you need, your team celebrates your progress and helps you transition to self-managed care with resources to stay supported long-term.
