Individuals can track progress, connect with their care team between visits and access on-demand, expertly crafted pain education and tools. These short sessions are built around neuroscience, mindfulness, and movement, helping build habits that help them feel better, move easier, and live more fully.

What Your Care Journey Looks Like

Month 1

Patients connect with therapists, physical therapists, and coaches based on their needs. The care team aligns on a unified plan that fits the individual’s life, goals, and symptoms.

Ongoing Support

Patients keep using the app to message providers, track goals, and access on-demand tools. Many individuals continue with booster visits or coaching even after graduation.

Week 1

Patients meet with a patient engagement specialist, share their story, and complete intake with a pain medicine provider. Together, the team begins building a personalized care plan.

Month 3

The team checks in on progress, refines the care plan, and helps patients stay on track. Whether individuals are building new habits or navigating setbacks, the team moves forward together.

Graduation

Once patients have built the skills and support they need, the team celebrates their progress and helps them transition to self-managed care with resources to stay supported long-term.
